There are many reasons for this stigma, which will be addressed a number of times throughout the book, but the most important thing is to understand that it is real and has to be explicitly acknowledged in counseling. Kennedy (2004) pointed out that service members are concerned that they will be stigmatized by seeking any type of counseling, so counselors must be able to assure the family of confidentiality. In addition, it is helpful for counselors to point out that rather than demonstrating weakness, seeking counseling is both a sign of strength and an act of valuing the family unit.

Concept of Honor It is to these last two points that Dr. Butler (personal communication, October 6, 2006) from San Diego warned female counselors that women may not understand the concept that getting killed is an honorable choice if it is in the carrying out of the mission. It might be compared to the mentality of the athlete that getting hurt is honorable if it is in the winning of the game. It might even be comparable to a mother risking her life for her children. But it is worth noting that the majority of counselors are women, and it might be more difficult for some female therapists to understand.

This distinction is important to understand, as there are two clear subcultures in the military, that of the enlisted and that of the officers. A noncommissioned officer (NCO) is someone who has risen from the lower ranks of the enlisted to enlisted officer status, but an NCO is still a member of the enlisted culture, not the commissioned officer culture. This has an impact not only on the service member but also on the family. Rank status in the military community is always visibly present and manifested by differentials in quality of housing and some social–recreational facilities, as an officer’s club or other facility (Rodriguez, 1984), even though these are generally on their way out on most installations.
